Job Description
As a Youth and Young Adult Advocate Case Manager, you will provide intensive case management services to youth ages 16–20 who are currently or formerly in foster care. This role focuses on helping clients achieve self-sufficiency by connecting them with housing, education, employment, healthcare, and community resources while maintaining compliance with program requirements and documentation standards.
Responsibilities
- Conduct client intakes, assessments, case planning, referrals, wraparound meetings, and ongoing case management services.
- Coordinate supportive services including housing, education, employment, healthcare, mentoring, and community-based resources.
- Maintain accurate case files and enter documentation into agency databases within required timelines.
- Prepare and submit weekly and monthly reports in compliance with program guidelines and contract requirements.
- Process requests for approved client assistance, including transportation, housing support, educational expenses, and other eligible services.
Qualifications and Requirements
- Bachelor's Degree in Education, Psychology, Sociology, Social Work, Criminal Justice, or another Behavioral or Social Science field.
- Minimum two years of paid case management experience working with youth in a social service setting.
- Experience maintaining case documentation and managing confidential client records.
- Valid Texas driver's license, reliable transportation, and current liability insurance.
- Strong interpersonal skills with the ability to build rapport and effectively work with youth in crisis or challenging situations.
