Job Description
The Service Coordinator supports residents and families by connecting them with resources, referrals, and case management services that promote employment, stability, and economic self-sufficiency. This role will conduct assessments, develop service plans, maintain documentation, coordinate with community partners, and provide follow-up support to ensure residents receive the services they need.
Responsibilities:
• Conduct resident interviews, needs assessments, and service plan development
• Provide case management support, including referrals, follow-ups, appointment setting, and crisis assistance
• Maintain accurate resident files, reports, case notes, referral forms, and related documentation
• Build relationships with community agencies, service providers, resident councils, and local resources
• Encourage resident participation in employment, training, and supportive service programs while tracking progress

Qualifications:
• Bachelor’s degree in Sociology, Psychology, Social Work, or related field preferred
• Five years of related case management experience with low-income individuals may substitute for degree
• Strong interpersonal, written, and verbal communication skills
• Ability to work independently, manage priorities, and maintain accurate documentation
• Valid Texas driver’s license, dependable transportation, and willingness to travel locally as needed
• Bachelor’s degree in Sociology, Psychology, Social Work, or related field preferred

• Strong interpersonal, written, and verbal communication skills

The base pay for this position is $23.57 per hour, excluding benefits, bonuses, or other compensation. Your final compensation will depend on your skills, qualifications, experience, location, and internal pay equity. Please note, hiring at the top of the range is uncommon to allow room for future growth.
