Job Description:
Provides nursing oversight and clinical support to individuals receiving services by assessing health needs, developing and implementing care plans, and coordinating care with members of the healthcare team. Maintains accurate medical documentation, monitors client conditions, and helps ensure quality care through ongoing assessment, education, and follow-up. Supports clients’ overall health and well-being through health monitoring, medication oversight, preventative care, and communication with families, staff, and providers.
Responsibilities:
- Conduct client health assessments to identify health needs and concerns.
- Develop, implement, update, and sign nursing care plans and client protocols.
- Maintain accurate medical records and documentation in HRST and Vypersoft.
- Access and update each client’s Health Risk Screening Tool (HRST) as medications or health conditions change.
- Provide nursing guidance to ill, injured, convalescent, disabled, or medically fragile clients.
- Educate clients, staff, and families on health needs, treatments, and wellness support.
- Monitor prescribed medications and document dosage, timing, and related information in client charts.
- Consult and coordinate with physicians, staff, and the healthcare team to assess, plan, implement, and evaluate care.
- Modify treatment and care plans based on client response and condition changes.
- Monitor and document symptoms, condition changes, diet, physical activity, and other aspects of client care.
- Observe nursing staff and client care practices to help ensure proper nursing services are provided.
- Prepare clients for examinations and assist with recommended treatments.
- Monitor healthcare needs, first aid, immunizations, and support during recovery and rehabilitation.
- Inspect client rooms, equipment, and supplies, and ensure needed medical and nursing supplies are stocked.
- Communicate with primary care providers to ensure clear and effective plans of care.
Qualifications and Requirements:
- Copy of RN license
- Copy of current TB test
- Copy of current physical exam, signed by a medical doctor
- Copy of diploma, GED, or degree
- Copy of valid driver’s license
- Copy of Social Security card
- Copy of current motor vehicle liability insurance
- Current CPR/First Aid certification
- Current Fire Safety certification

The pay range for this position is $40/hour, excluding benefits, bonuses, or other compensation. Your final compensation will depend on your skills, qualifications, experience, location, and internal pay equity. Please note, hiring at the top of the range is uncommon to allow room for future growth.
