Job Description:
Licensed Practical Nurses function in a dependent role under the direction of a Registered Professional Nurse or other authorized healthcare provider, as permitted by law. Under such direction, the Licensed Practical Nurse may administer medications, provide nursing treatments, and collect data on patients’ signs and symptoms to assist the Registered Nurse in making decisions regarding individual care.
Responsibilities:
- Collect data and assist with health assessments.
- Collaborate with the RN or APP in developing and updating care plans and carry out delegated nursing tasks.
- Provide health teaching and counseling.
- Advocate for individuals by communicating and collaborating with healthcare providers and other service personnel, as delegated.
- Participate in evaluating each individual’s response to treatments and interventions.
- Provide input into policies and procedures that support individual health and safety.
- Administer and review medications according to physician orders.
- Ensure Medication Administration Records (MAR) are accurate and updated as needed.
- Ensure prescriptions are current, uploaded, reviewed annually, and followed appropriately.
- Order medications at least 7 days before they run out and pick them up when needed.
- Ensure proper storage and destruction of medications and document all destroyed medications on the appropriate log.
- Monitor seizure activity, signs of illness, fall risk, ambulation concerns, nutrition, bowel function, urinary status, and skin integrity.
- Monitor for changes in weight (If Congestive Heart Failure be sure to include heart and lung sounds)
- Perform preventative measures to reduce skin breakdown and check daily for injuries or other concerns.
- Take and document vital signs, including blood pressure, pulse, and respirations.
- Perform head-to-toe assessments.
- Complete HRST data tracking in VRST.
- Review appointments, lab results, hospital discharge notes, and follow-up care to ensure appropriate action has been taken with primary care providers, specialists, and other services.
- Complete appointment education sheets following appointments.
- Enter appropriate skilled nursing notes in Vypersoft modules.
- Perform other duties as assigned.
Qualifications and Requirements:
- One to three years of training and/or experience in a hospital, private duty nursing, mental health, or related setting preferred
- At least six months of experience providing direct care or social services to individuals with special needs preferred
- Ability to work independently
- Basic computer skills, including email, internet, and data entry
- Copy of LPN license
- Copy of current TB test
- Copy of current physical exam, signed by a medical doctor
- Copy of diploma, GED, or degree
- Copy of valid driver’s license
- Copy of Social Security card
- Copy of current motor vehicle liability insurance
- Current CPR/First Aid certification
- Current Fire Safety certification
Additional Qualifications:
Successful completion of a practical nursing program approved by the Georgia Board of Nursing. Must have passed the National Council Licensure Examination for Practical Nurses (NCLEX-PN) and hold a current Georgia LPN license. Must also complete orientation and competency-based training required by the provider agency and the State of Georgia.

The pay range for this position is $30/hour, excluding benefits, bonuses, or other compensation. Your final compensation will depend on your skills, qualifications, experience, location, and internal pay equity. Please note, hiring at the top of the range is uncommon to allow room for future growth.
