Job Description:
Provide compassionate support services to adults with learning disabilities while promoting independence, self-awareness, and personal growth. Support residents with daily living activities in a safe, respectful, and supportive environment that values each patient’s needs.
Duties:
- Assists clients with working towards goals identified in the Individual Support Plan (ISP) and documents progress.
- Completes daily progress notes highlighting person-centered events and other required documentation. Ensure all documentation of services, are accurate, timely and in compliance.
- Observes and reports to your immediate supervisor any changes in the client's condition and/or behavioral changes. Documents occurrences in client records.
- Assists with and encourages personal hygiene and bathing, oral hygiene and dental care, grooming, shaving, hair care, elimination, toileting hygiene, ironing client's clothes and dressing.
- Prepare a well-balanced meal for breakfast, lunch, snack and/or dinner. Monitors dietary restrictions of clients and ensure proper and safe nutrition. Assists with feeding clients, as needed.
- Assists with meal planning, shopping, and bill paying and errands as needed.
- Takes vital signs when assigned, observes and reports changes in a client's condition, including behavioral changes. Documents occurrences in client records. Arranges trips to the doctor, picks up prescription drugs from pharmacy, accompanies clients on medical appointments, documents client's food and/or liquid intake or output, reminds clients to take medication, and assists with self-administration of medication. Activities shall not include professional services that are subject to regulation under professional practice and licensing.
- Monitors environment for safety, inspects house daily for hazards encourages safety measures, and advises in the correct operation of household appliances. Report any safety concerns to the supervisor.
- Assist client with safe ambulation and transfer.
- Perform activities of daily living assistance to include, but not be limited to washing dishes, laundry, changing linens, trash disposal, cleaning, vacuuming, and cleaning bathrooms.
- Attends all staff meetings, participates in scheduled training sessions and on the job training. Ensures annual training requirements are met.
- Promotes harmonious and cooperative relationships with other staff members.
- Maintains client records on site in accordance with policy and procedures.
- Always represents Company in a professional and ethical manner.
- Supports and abides by all company policies and procedures.
- Performs other duties as assigned.
Qualifications and Requirements:
- 6-months of experience providing social or direct care services to groups of special needs individuals in a human service setting
- Copy of CNA certificate, if applicable
- Copy of current TB test
- Copy of current physical exam, signed by a medical doctor
- Copy of diploma, GED, or degree
- Copy of valid driver’s license
- Copy of Social Security card
- Copy of current motor vehicle liability insurance
- Current CPR/First Aid certification
- Current Fire Safety certification

The pay range for this position is $14/hour, excluding benefits, bonuses, or other compensation. Your final compensation will depend on your skills, qualifications, experience, location, and internal pay equity. Please note, hiring at the top of the range is uncommon to allow room for future growth.
