Job Description:
A public health organization is seeking an experienced Public Health Nurse IV to provide tuberculosis (TB) nurse case management services. This role plays a vital part in coordinating patient care, ensuring compliance with treatment plans, and supporting community health outcomes through accurate reporting and collaboration.
A public health organization is seeking an experienced Public Health Nurse IV to provide tuberculosis (TB) nurse case management services. This role plays a vital part in coordinating patient care, ensuring compliance with treatment plans, and supporting community health outcomes through accurate reporting and collaboration.
Responsibilities:
- Provide comprehensive tuberculosis nurse case management
- Coordinate services including transportation and medical eligibility assistance
- Complete and submit required clinical and program reports
- Collaborate with internal and external partners to ensure continuity of care
- Perform other duties as assigned to support public health operations
Qualifications and Requirements:
- Graduation from an accredited school of nursing; Bachelor of Science in Nursing preferred
- Current and valid Registered Nurse license in the State of Texas
- Minimum of four years of experience as a practicing registered nurse
- Master of Science in Nursing plus two years of general nursing experience, including one year of supervisory experience, may substitute for the required experience
- Valid Texas Driver’s License and compliance with AP 2-2 policy
- Ability to meet all Houston Health Department Temporary Employee vaccination requirements
